This is a Walk-UP Vaccine clinic offering the Pfizer Vaccine and the Johnson & Johnson Vaccine, located at the Trinity United Methodist Church, 4000 NW 53rd Ave, Gainesville, FL 32653. This event is being held on Monday May 24th from 10am to 2pm.

Important Information:
Effective April 5, 2021, all Floridians shall be eligible to receive any COVID-19 vaccine as prescribed by the Food and Drug Administration.
For residents who are 12-17 years old, we are offering only the Pfizer vaccine. Please take note, the following procedures must be observed: All individuals 12-17, a parent/guardian must be present at the time of vaccination and sign COVID-19 vaccine consent form.
To expedite your visit, download and fill-out the appropriate consent form and bring with you to this event. If you are unable to download this form and print it out, forms will be avaialable for you at the event. Read and review the appropriate Fact Sheet and Notice of Privacy Practices.
