This is a Walk-UP Vaccine clinic offering the Pfizer Vaccine, located at the Trinity United Methodist Church, 4000 NW 53rd Ave, Gainesville, FL 32653. This event is being held on Tuesday June 1st from 9am to 4pm.


Important Information:


For residents who are 12-17 years old, we are offering only the Pfizer vaccine. Please take note, the following procedures must be observed: All individuals 12-17 years old, a parent/guardian must be present at the time of vaccination and sign COVID-19 vaccine consent form.

To expedite your visit, download and fill-out the appropriate consent form and bring with you to this event. If you are unable to download this form and print it out, forms will be avaialable for you at the event. Read and review the appropriate Fact Sheet and Notice of Privacy Practices.
