This is a Walk-UP Vaccine clinic offering the Pfizer and Johnson & Johnson Vaccine, located at the Trinity United Methodist Church, 4000 NW 53rd Ave, Gainesville, FL 32653. This event is being held on Tuesday June 8th from 9am to 3pm.
Important Information:
Effective April 5, 2021, all Floridians shall be eligible to receive any COVID-19 vaccine as prescribed by the Food and Drug Administration. State Surgeon General Scott A. Rivkees issued a Public Health Advisory on January 21, 2021 prioritizing vaccines for residents and seasonal residents of Florida in conjunction with Executive Order 21-79.
To receive the COVID-19 vaccine in Florida, you must bring proof of residency. Please see this residency notice for the requirements needed to prove residency.
For residents who are 12-17 years old, we are offering only the Pfizer vaccine. Please take note, the following procedures must be observed: All individuals 12-17 years old, a parent/guardian must be present at the time of vaccination and sign COVID-19 vaccine consent form.
To expedite your visit, download and fill-out the appropriate consent form and bring with you to this event. If you are unable to download this form and print it out, forms will be avaialable for you at the event. Read and review the appropriate Fact Sheet and Notice of Privacy Practices.
