All children ages 6 weeks through age 18 can receive free childhood immunizations. Anyone ages 19 and older may qualify for another free selection immunization program. If a client does not qualify, there will be a fee for immunizations for those ages 19 and older. Federal regulations require the Florida Department of Health in Alachua County (DOH-Alachua) to collect insurance information for all clients, regardless of age.

School Immunization Requirements
Childcare and/or Family Daycare
- Diphtheria, tetanus, and acellular pertussis (DTaP)
- Inactivated polio vaccine (IPV)
- Measles, mumps, and rubella (MMR)
- Varicella (chickenpox)
- Haemophilus influenzae type b (Hib)
- Pneumococcal conjugate (PCV15/20)
- Hepatitis B (Hep B)
Preschool Entry
- DTaP
- IPV

Kindergarten through 12th Grade
- 4-5 doses of DTaP
- 4-5 doses of IPV
- 2 doses of MMR

- 1 tetanus, diphtheria, and acellular pertussis (Tdap)
- 2 doses of varicella
Kindergarten effective with 2008–09 school year, then an additional grade is added each year thereafter. Vaccine is not required if there is a history of varicella disease documented by a health care provider.
Additional Requirements: 7th Grade
In addition to compliance with all other immunization requirements, children entering, attending, or transferring to the 7th grade in Florida schools must complete the following:
- One Tetanus-diphtheria-acellular pertussis (Tdap)
Documenting Immunizations
The Form DH 680, Florida Certification of Immunization, must be used to document receipt of immunizations required for entry and attendance in Florida schools, childcare facilities, and family daycare homes.

Florida SHOTSTM
The Florida State Health Online Tracking System is a free, statewide online immunization registry that helps health care providers, schools, and parents keep track of immunization records. A Florida SHOT Form DH 680, certified with electronic signature, may be printed by enrolled health care providers and is also accessible to any school, licensed childcare facility, or daycare center enrolled in Florida SHOTS. Parents can also access their child’s certified Form DH 680 with a personal identification number issued by your child’s health care provider.
