Every child entering a Florida school for the first time needs the Florida Certification of Immunization form (also known as Form 680 or “blue card”). This form must be completed by a physician or a Florida county health department. Please note: Form 680 may be printed on blue or white paper; as long as the form is certified, it is an official form.

Submit Out-of-State Records for Transfer to Alachua County

Below is the process to submit immunization records for transfer. There is a $10 fee due upon submission of forms to complete the transfer. A current working phone number is required to be entered on the immunization records transfer form or transfer will not be processed.

Submit documentation by one of the following options:

  • Option 1: Visit our online portal and complete all required fields – FL Immunization Screening Portal
  • Option 2: Drop documents off in person at the East Gainesville Clinic or the Alachua Clinic
  • Option 3: Fax documents to DOH-Alachua at 352-334-7943
  • Option 4: Mail documents to:
    Florida Department of Health in Alachua County
    ATTN: Immunizations
    224 SE 24th St.
    Gainesville, FL 32641

If choosing option 2, 3, or 4 you are required to complete and submit the following information to DOH-Alachua at least three weeks prior to school orientation/registration:

Keep copies for your records; never submit original documents. Make sure writing is legible.

All records will be processed within 48 hours of receipt. A nurse will notify parent/guardian when record is entered into FLShots and advise parent/guardian if child will need additional vaccines. Illegible/incomplete patient information will not be processed.

Copies of immunization records can be picked up in person at DOH-Alachua office. Records cannot be faxed or emailed back.

School Age Immunizations Records Request

Parents and/or guardians of those under the age of 18 can request a copy of their child’s immunization records for vaccinations recorded in the Florida State Health Online Tracking System (FL SHOTS) by one of the following options:

  • Option 1: Contact your child’s primary care provider. 
  • Option 2: Visit our online portal and complete all required fields – FL Immunization Screening Portal
  • Option 3: Visit your local health department and request your historical recods. Photo ID is required. 

To obtain a certified Form 680 it must be requested as named in the portal. Child may need to be evaluated if not up-to-date on immunization schedule. School Entry Health Examinations (Form DH 3040) are by appointment only.

Adult Immunizations Records Request

Adults (aged 18 and over) can request a copy of their immunizations historical records for vaccinations recorded in FL SHOTS by one of the following options:

  • Option 1: Contact your primary care provider. 
  • Option 2: Visit our online portal and complete all required fields – FL Immunization Screening Portal
  • Option 3: Visit your local health department and request your historical recods. Photo ID is required.
