All proposed new installations, repairs, modifications, and approvals of existing Limited Use Public Water Systems require a permit from the Florida Department of Health in Alachua County. Applications for permits are made at DOH-Alachua.

Fees
- Limited Use Well Construction Application (Serves as 1st year permit): $90
- Limited Use System Annual Operating Permit: $90
These fees do not include the required clearance or compliance sampling. DOH-Alachua can provide these services at an additional cost. Please contact us for additional information regarding these services.
